Under each tier level you will not pay more than $250 per prescription order or refill

  • Tier 1 | $20 Copay A generic drug is a medication created to be the same as an already marketed brand-name drug in dosage form, safety, strength, route of administration, quality, performance characteristics, and intended use.
  • Tier 2 | $40 Copay
  • Tier 3 | $75 Copay

Under each tier level you will not pay more than $750 per prescription order or refill

  • Tier 1 | $50 Copay A generic drug is a medication created to be the same as an already marketed brand-name drug in dosage form, safety, strength, route of administration, quality, performance characteristics, and intended use.
  • Tier 2 | $100 Copay
  • Tier 3 | $187.50 Copay

To maximize your Benefit, ask your Physician to write your Prescription Order or Refill for a 90-day supply, with refills when appropriate. You will be charged a mail order Co-payment for any Prescription Orders or Refills sent to the mail order pharmacy or Preferred 90 Day Retail Network Pharmacy regardless of the number-of-days supply written on the Prescription Order or Refill. Be sure your Physician writes your Prescription Order or Refill for a 90-day supply, not a 30-day supply with three refills.
